The Lancet (2022-06-04). [Editorial] Gun violence in the USA: children's right to health. thelancet.com An epidemic of preventable violence is killing and harming Americans—especially, children and adolescents. On May 24, an 18-year-old man killed 19 children and two adults in Robb Elementary School in Uvalde, Texas. As the world mourns for the US and its children, the familiar cycle of grief, anger, activism, deflection, and inaction seems to have begun again. We share the view of those who argue that gun violence should be framed as a public health issue rather than a debate over the US Constitution's Second Amendment.

Pratik Chakrabarti (2022-06-04). [Perspectives] Health as activism: rethinking social medicine in India. thelancet.com The COVID-19 pandemic has exposed deep problems and inequities in India's health infrastructure, which has suffered from decades of underinvestment. But the pandemic also galvanised a wide range of community participation. Non-governmental organisations (NGOs), volunteers, and health-care workers arranged for the supply of oxygen, helped register people for vaccination, ran vaccination camps, and visited communities and schools to speak about the virus and precautions. Notably, many local women, who trained as accredited social health activists (ASHAs), took part in these efforts.

Richard Horton (2022-06-04). [Comment] Offline: WHO's erasure of Palestinians must cease. thelancet.com Global health is a graveyard of good intentions. WHO's Director-General, Tedros Adhanom Ghebreyesus, gave a widely admired address to the World Health Assembly last week. He spoke of "the profound consequences of conflict" for health and concluded his speech with these words: "Unless we seek peace, we will find war. Today, and every day, we have a choice—we make the choices. And today, and every day, we must choose health for peace, and peace for health. Peace, peace, peace." One choice his agency has made is to erase the existence of over 5 million people from its "comprehensive" assessment of world health…

[Correspondence] Head and neck cancer: high-end technology is no guarantee of high-quality care — Authors' reply. thelancet.com We thank Johannes H A M Kaanders for commenting on our Seminar in which we discussed recent advances in the management of head and neck squamous cell carcinomas.1 We agree with the need to exert caution in the wide and uncontrolled implementation of novel modalities such as transoral robotic surgery and proton therapy. We also agree with the need to generate confirmatory data for these modalities Both transoral robotic surgery and proton therapy are tools with the potential to refine and advance existing (approved) modalities.

Fay Bound Alberti (2022-06-04). [Perspectives] Harold Gillies and the battleground of surgical innovation. thelancet.com The damaged faces of soldiers treated by Harold Gillies have become something of a staple of popular culture since the World War 1 centenary. Thus, "shocking pictures" of "horribly disfigured" servicemen have featured in British tabloids, their faces assembled into a jarring photo-mosaic, showing a damaged mass of missing jaws and blown-away noses before being salvaged, corrected, and filled in by the pioneering New Zealander and so-called father of plastic surgery. Historians of medicine have recounted how the case photographs of the patients Gillies treated have even made their way into video games.

[Articles] Intrauterine device-related uterine perforation incidence and risk (APEX-IUD): a large multisite cohort study. thelancet.com Although the risk for uterine perforation with IUD insertion 4 days to 6 weeks or less post partum is nearly seven times that of insertion non-post partum, perforation remains an incredibly rare event for all clinical time points. Despite a slight increased risk of perforation with breastfeeding at IUD insertion, the benefits of breastfeeding and effective contraception generally outweigh risks and should have little clinical impact. Therefore, IUD insertion timing should be based on individual desire for IUD contraception and patient convenience to assure an IUD insertion can occur.

Deborah Bartz (2022-06-04). [Comment] Incidence of perforation with intrauterine devices. thelancet.com When available and used properly, intrauterine devices (IUDs) are near-perfect birth control as they provide highly effective protection against pregnancy with minimal to no adverse systemic side effects. Therefore, patient satisfaction rates are high.1 Given these successes, IUD provision has markedly expanded.2,3 In many contraceptive markets globally, new IUD doses and sizes have been added recently. Several devices are indicated for extended use beyond their labelled duration, for post-partum and post-abortion placement,4 for emergency contraception,5 for use in people at extremes of reproductive age, for pre…
