[Articles] Opioid versus opioid-free analgesia after surgical discharge: a systematic review and meta-analysis of randomised trials. thelancet.com Findings from this meta-analysis support that opioid prescribing at surgical discharge does not reduce pain intensity but does increase adverse events. Evidence relied on trials focused on elective surgeries of minor and moderate extent, suggesting that clinicians can consider prescribing opioid-free analgesia in these surgical settings. Data were largely derived from low-quality trials, and none involved patients having major or major-complex procedures. Given these limitations, there is a great need to advance the quality and scope of research in this field.

The Lancet (2022-06-18). [Editorial] Attitudes towards menopause: time for change. thelancet.com In this week's issue of The Lancet, a Comment by Zoe Schaedel and Janice Ryder highlights many of the misconceptions around, and barriers to, menopause care. Stigma, embarrassment, a lack of public awareness, and miscommunication mean that for many women, menopause is either borne in quiet suffering or else over-medicalised as nothing more than a hormone deficiency requiring oestrogen replacement. Menopause is a natural part of ageing that affects 50% of the population. Yet, this fact is not reflected in many societies, and their health systems.

Alberto Mussetti, Anna Sureda (2022-06-18). [Comment] Second-line CAR T cells for lymphomas. thelancet.com Anti-CD19 chimeric antigen receptor (CAR) T cells have shown huge therapeutical potential in the treatment of aggressive B-cell lymphomas beyond second-line of therapy.1—3 But the use of such therapies as a second-line of therapy is a current matter of debate. At the 2021 American Society of Hematology meeting, results of the three randomised clinical trials addressing this question were presented. The ZUMA7,4 BELINDA,5 and the TRANSFORM6 trials included only patients with aggressive B-cell lymphomas with primary refractory disease or with relapse within 12 months of completion of first-line therapy.

Danielle Ofri (2022-06-18). [Perspectives] Memorialising COVID-19. thelancet.com The notifications are more than 2 years old by now, but I still cannot bring myself to delete them from my hospital's electronic medical record (EMR). There is the "expiration alert" that signalled my very first patient to die of COVID-19—a chipper Ecuadorian who had spent months struggling to recover from profound depression, only to fall victim to SARS-CoV-2 in the first weeks of the pandemic. There is the desperate note from a daughter whose father with liver disease was decompensating rapidly at home, but was too afraid of COVID-19 to seek medical attention.

Paul S Myles, Thuy Bui (2022-06-18). [Comment] Opioid-free analgesia after surgery. thelancet.com Acute pain is near universal after surgery. Opioids have been a mainstay of postoperative analgesia, but current best practice emphasises a multimodal opioid-sparing regimen based on a background of paracetamol, a non-steroidal anti-inflammatory drug, or both.1 Opioids are frequently overused, particularly after hospital discharge.1—3…

Richard Horton (2022-06-18). [Comment] Offline: WHO's hits and misses for mental health. thelancet.com There are few truly watershed moments in medicine and global health. But the publication this month of WHO's World Mental Health Report is one such milestone. It is the agency's first major global foray into mental health for over two decades. Led by Dévora Kestel, who directs the Department of Mental Health and Substance Use in Geneva, WHO aims, in her words, for nothing less than "a transformation" in mental health. 20 years ago, mental health was completely ignored by development experts. There was no mention of mental health in the MDGs.

Vivian B Pender (2022-06-18). [Perspectives] Psychiatry in crisis: great expectations, elusive cures. thelancet.com There is no denying an ongoing crisis in mental health. For individuals and those around them, persistent mental illness can have devastating impacts. Yet, there is a substantial gap between people who need mental health care and access to effective treatment. Even when psychiatric care is accessed, those affected might not find the help they need. Two recent books address psychiatry's efforts to manage the crisis. [Comment] Relugolix combination therapy for endometriosis pain. thelancet.com The treatment of endometriosis-associated pain continues to be a major problem for women and their clinicians.1,2 Laparoscopy, required to confirm the diagnosis, is usually combined with surgical ablation or excision of endometriotic lesions to alleviate pain and hormonal contraceptives are used to control symptoms before and after the operation.1,3 Recurrence of pain is common, with 40—50% of women with endometriosis reporting recurrence within 5 years of treatment4 and nearly two-thirds of women requiring repeat surgery.
